The explosion occurred on Saturday and targeted Pakistan's minority Shia population as dozens were shopping at a crowded vegetable market in Quetta, local officials said.
Rescue workers and victims' families are still identifying bodies and the death toll is likely to rise further, local community leaders told Al Jazeera.
The suicide bomber vehicle was packed with some 800 kilos of explosives and caused nearby buildings to collapse, officials said.
For years Sunni Muslim extremist groups have targeted Shiites in Pakistan. In early January, 92 people were killed and more than 120 injured in twin suicide bomb blasts in Quetta.
